By blocking the domains at an ISP level. It would only affect one country.

Even if an ISP uses DNS blocking there are still ways to access the blocked site, A few that come to mind are...

1, Use another DNS server.
2, Use a proxy.
3, Use a VPN.
4. Could even just use the IP address directly if known for the site that is blocked

Of course, wee would know this but the majority of Americans would not.

Obama actually noted 3rd party options in his statement on why SOPA didn't work - the fact that it would lead Americans to potentially unsafe options which is true.
