I had an issue like that when I was making a large icon theme a couple years ago. Your icons are too big and using too much resources and that's why they won't display. Try minimizing the sizes before injecting them into the RCO. A very crude but VERY effective way of doing it is by opening your image editing software ( I used GIMP ) and im assuming your icons are already saved as .PNG . What you want to do is save each one as a .BMP with 16bit or 8bit (whichever is the smallest option ) image properties ( on GIMP the option will show up after you click save as , then just select additional options ) then take the newly saved .BMP icon you just saved and re-open it and then save it as .PNG again and then inject. My theme the icons covered the whole screen and had 90% transperancy ( Google "psp theme + Borders" and you will see what I'm talking about ) and once I did this all of them showed up with no problems. Nice layout I like it, Hopefully this helps.
