I think he wants to add another main category in the xmb.
Main categories wee already have are SETTINGS, PHOTO, MUSIC, VIDEO, GAME, NETWORK, & PSN.

As far as I know this is not possible with the current tools wee have.
It could maybe be possible if one had the knowledge to create a custom firmware with this feature.
It might even be able to be accomplished with a custom written plugin.

I could however, offer you a few easy alternatives:

1) You could add a fake, non-functional, non-highlightable, main category.  You could do this by doubling the width of the canvas for either your SETTINGS or PSN icon.  Then just add the other icon to the respective outside of that icon.  

This however seems cheesy to me.  So I would suggest idea number two:

2) Rearrange the icons by altering vshmain.prx.  Make your own categories out of the seven slots available.  Move what you want where you want it.  Then edit the topmenu_icon.rco to customize the icon for your categories.  The, finally, use the new XMBIC plugin to make the new icons function the way you want them to.  Have them launch whatever you want.  You can even change the names of them.  (You could also do that by editing topmenu_plugin.rco, but that is not necessary with XMBIC plugin, b/c this is a built in feature)

I know that's a lot of stuff i just threw out there.  
I think option #2 is the best.  It will not allow you to create another category, but it will allow you to hijack and fully customize the current categories.

Use dynamic editor and open a decrypted vshmain.prx.  
You will see how you can move icons to new categories with the program.

You can get the prx files, and dynamic editor right in the theme forums   ;)
