I've also seen S rank in Devil May Cry, and Shinobi(?), if I remember correctly. Or the term "S-class" used in anime to describe a super powerful enemy, or someone who is very skilled at something.

F for Failure
E for Effort
E is after F in games and it's the worst grade you can get.
I always thought it stood for special or super.
