It only works if you have a PlayStation Account. If somebody could take screenshots of every page and post it I would like it. I already completed the survey and cannot do it again.

That link isn't working for me, but people are saying the actual survey is about 20 pages long...

Okay, I am giving up. I've spent the last 40 minutes or so looking for someone who was smart enough to use a method to save a record of the whole survey, but can't find anything.
