I have never used chrome so obviously this is guess work from reading a little...

I read that Alt+W mutes/unmutes all tabs in google chrome but I'm not sure if this is true as I could not find the shortcut in the google chrome docs.  (maybe it's with a plugin?)

Maybe worth a try...

So it's not Flash then, it's Chrome. That was an HTML5 video which didn't use Flash for playback.

When you right click the audio icon in your tray, click "Playback Devices" - let us know what devices are listed. Make sure to right click the list and tick "Show Disabled Devices" and "Show Disconnected Devices" as well.

Does it not work with or without headphones connected?

Here the video was playing and I ticked "Show Disabled Devices" and "Show Disconnected Devices"

Well that indicates it's using Windows drivers.. how strange. Check devices when you plug in the headphones and see if the device changes - also, when you play a video in Chrome check the meter which indicates sound playback (on the right) in that panel with the sound devices and see if any of the listed devices seem to indicate Chrome's audio is being played on one of them.

Uninstall Google Chrome and your sound drivers. Open device manager and click on Sound, video and game controllers. Uninstall your sound driver. Reboot after that and Windows should install the drivers again. Then install Google Chrome and hope for the best.

If that doesn't work disable your antivirus/firewall/malware stuff...could be that idk. =I
