Most fansub groups do sign (referring to any Japanese words that appears in the picture) translations.... to a degree.  Usually basic .donkey typesetting for the important signs, for normal groups.  However, the most fancy (read: herp derp I'm better than the rest) ones use AFX (Adobe After Effects), and sometimes do it for every bit of Japanese that they can visibly see... and that's why it sometimes takes them like forever to release anything (Menclave is a classic example of this).
