If there's 3 clinics, surely there's plenty of other doctors there already, right? Then you could simply be the manager as stated, and have a few more doctors fill in, if necessarily. Management is not necessarily easy though.

You could even hire a manager to do all the work actually, though this kinda gets tricky.
