@nicodemus82: as warfab said, i tried to build the ptf with v1.0.0.0, icon/preview are fine.
for your dAppleTV attached, missing topmenu_plugin.rco, makes incorrect text label location

The one from http://www.jp.playstation.com/psp/dl/too..._Theme.zip is v1.2.0.0

I think I got my v.1.0.0.0 from the US or EUR PSP download (they didn't have the latest version yet when I downloaded from them).
