I used the same paf.prx & vshmain.prx from Haunter Remixedv4.5
man, those are a lot of screenshots!
I noticed that the custom click sounds or at least the "cursor" custom click sound doesn't play when scrolling through the XMB if anyone knows how to fix that could you please leave a comment? I recently found out that the paf.prx and vshmain.prx came from a theme called "GraffitiMontage" which I used in both the Haunter Remixedv4.5 and Paper Mario (Mario Story Glory) themes. Whoever made that theme big thanks to you I love the layout and will continue to use it for upcoming themes.
Have no clue on the answer, and PSP is somewhat messed up to test, but have you tried making the sounds mono?
Maybe try swapping VAGs from a working theme and see if that works.

Hey thanks for answering ZiNgA, I haven't tried making the sounds mono. I don't have the program "SONY Sound Forge" installed anymore I had to image my computer back to a previous state a couple days ago. I think once I get that program installed again I'll give it another whirl.
If all you need a sound editor for is to convert stereo » mono, you don't really need Sound Forge.  Something like Audacity should be able to do the trick.

Alternatively, dump the RCO with rcomage (don't convert VAGs), and edit the XML to say there's only one channel for the audio.  After saving the XML, compile it into an RCO.
Setting the channel count to 1 will just take the left (I think) channel.

When I get in the theme making mood again I'll give that a shot, not too sure on how to edit the RCO though but I'm sure I could figure it out, and you're welcome (for the theme I mean). This site has taught me so much I like to give back.
