you could also try to add the icons "uncompressed" go into options in RCOedit and check the box "don't compress images and models"

not sure if this helps but did for me
if i use this option the rco gets corrupt after adding the 3rd image
rco editor also give me an error message

what do you mean by restoriung the default?
I mean you replace the original topmenu_icon.rco in order for you to verify the issue. Normally, you encounter "missing" icons if you have a topmenu_icon.rco having too many big images. I am assuming that you're not falching high resolution 01-12.bmp too.
